View in Terms of ServicePlease provide at least 24 hours' notice for cancellations whenever possible. Cancellations should be submitted by email to [email protected].
Cancellations made with less than 24 hours' notice may be subject to a $50 inconvenience fee. This fee covers staff time, travel preparation, scheduling, and work already performed in preparation for the appointment.
No-shows — meaning the client is unavailable, unreachable, or unable to proceed at the time of the scheduled appointment — may result in forfeiture of part or all of the service fee, depending on the service type and preparation already completed.
Same-day, urgent, VIP, specialty kit, DNA, employer-requested, chain-of-custody, or facility-based appointments may have stricter cancellation terms due to preparation, scheduling, compliance, or logistics requirements.
If additional tests or services are added after the original booking, additional service fees may apply. These fees are separate from any cancellation or inconvenience fees.
If you need to reschedule rather than cancel, please submit a reschedule request by email to [email protected] if the online portal is unavailable. Include your name, original appointment date and time, service address, and your preferred new date and time.
Elite Phlebotomy Concierge reserves the right to cancel an appointment due to unsafe conditions, severe weather, missing documentation, payment issues, service limitations, client unavailability, inappropriate conduct, or operational needs.
If Elite Phlebotomy Concierge cancels an appointment for reasons unrelated to client conduct, missing information, or documentation issues, we may offer a reschedule or refund based on the circumstances.
